- Notary Document:
- Separate Document: If your notary document is separate, write “See attached for notary form” at the bottom of the parental authorization document. Attach the notary document with the form.
- Details: The notary document should mention the name of the parental authorization document i.e. Parental Authorization for Minors and include both parents’ names.
- Married Parents:
- Applicability: These instructions apply when both parents are married to each other.
- Signatures: Both parents should sign the parental authorization form.
- Supporting Documents:
- Capacity Section: For the text on the form under “Capacity”, which states that both parents or guardians should provide a copy of their ID cards with signatures and the child’s birth certificate: all these documents are already part of the physical document list.
